The City of Kirkland became a Passport Acceptance Facility in January of 2005, allowing us to assist with applications for a US passport. By US regulation, only federal, state or local government officials may serve as passport acceptance agents. Acceptance agents receive applications from the public and review them to ensure all necessary information and citizenship evidence is included. They administer the required oath and witness the applicants' signatures, then forward the applications to the Passport Services Agency (US Dept. of State) for issuance. Acceptance facilities also provide the public with general information on passport procedures and serve as a liaison between applicants and Passport Services Agencies. |

The actual decision to issue or deny a passport is made only by a Passport Services Agency, not the City of Kirkland. To check on the status of a submitted passport, please call Toll Free 1-877-487-2778.

What do you need to apply?
Original Documents Only
- Passport Application (Form DSP-11, can be obtained at City Hall or www.travel.state.gov
[see below for information on Renewals, Form DSP-82])
- Two identical photographs (2" x 2")
- Proof of citizenship:
- Certified copy of birth certificate issued by the state, county, or city: OR
- Previously-issued passport (if you have a previous passport it must be submitted with the application); OR
- Certificate of Naturalization - no photocopies
- Photo identification such as:
- Driver's license (no state ID); OR
- Government employee I.D. card; OR
- Official Military I.D. card; OR
- Aircraft pilot's license issued by DOT, FAA
(We cannot accept social security or Medicare cards, credit cards, voter registration, or club membership cards.)
**IMPORTANT NOTICES**
Effective February 2008
- If a child is under age 16, parents, custodians, and/or legal guardians may apply on the child's behalf.
- All children must appear in person.
- Children under 16
who have had a prior passport must provide both passport and birth certificate to show citizenship and parental relationship.
Effective November 1, 2004
The Statement of Consent (form DS-3053, or same information provided on other paper) by non-applying parents for children under age 16 must be notarized.
Passport Renewals
The City of Kirkland is not authorized to process Passport Renewals; they must be done by MAIL. (You may obtain the form at City Hall or www.travel.state.gov
.) You may apply for a passport renewal only if you meet the following criteria, otherwise you must apply for a new passport:
- You can submit your most recent passport; AND
- Your previous passport was issued on or after your 16th birthday; AND
- Your most recent passport was issued less than 15 years ago; AND
- You use the same name as on your most recent passport, OR have had a name change by marriage or court order and can submit proper documentation to reflect the name change

TIMELINE: It takes approximately four to six (4-6) weeks to receive a passport. Expedited service is available for an additional fee and takes approximately two to four (2-4) weeks. It may be possible to improve these timelines by paying for Express Mail. Expedited service may be necessary if you have immediate travel plans, or if you are going to a country that requires a visa for travel (you must have your passport in hand before you can apply for a visa).

Passport Fees
Passport fees must be paid as separate payments:
- $25.00 cash, check, or money order made out to the CITY OF KIRKLAND for the processing fee.
- Check or money order only made out to US DEPARTMENT OF STATE for the passport fee, security surchage and any expedited services fees.
- Each passport application must be paid with its own check or money order.
